I first saw this at the tumblr Book Patrol; they corrected their post fairly quickly.
The book, Lee Server’s Baby I Don’t Care, a biography of Robert Mitchum, and its annotations, belong to Tony Shafrazi—
—and—
Enoc Perez took the photo–
The novelist James Boice seems to be the origin of the link between the Mitchum biography to DFW/JJ (clearly a jest):

And then somehow the pic got to tumblr.
Oh, and, here’s a bigger pic of the passage from Server’s book:
I initially posted this in fanboyish excitement—despite no real legitimate source (like the HRC archive). The image is too tiny to reveal much when enlarged, but that doesn’t look like Ulysses. And Wallace’s supposed annotations contrast the general style we can observe in his archival material.
So sorry. Posted this, and updated the title with parentheses in a ten minute span.

And then updated again just now, changing the title again…See @erasing’s tweet.

Since I seem to be using my comments as an update space:
The original version of this post featured the tumblr image, which was fairly low res; I’ve since updated to feature the photograph by Enoc Perez of Tony Shafrazi’s book/annotations/artwork (source: http://instagram.com/p/mGk7H7shMu/ ).
